Step 2. Download Services Agreement, Non-Disclosure Agreement and W-9.
Step 3. Fax us the following documents at 813-968-5622.
1. Signed Enrollment form and Agreements
2. Copy of valid Notary Commission/ Certificate (and Attorney Bar card if applicable)
3. Copy of E and O Insurance
4. Two references (or upload resume and references on next page)
5. Completed W-9
Step 3. Once your application has been processed and approved, you will receive email notification that you have been added to our database.
